Low Cost Athletic Physical Exams
Rehab United is hosting a High School Physicals night on Monday, May 21st at 6pm. Your cost is $20 and the proceeds are donated back to the school to help defer the cost of our Athletic Trainer. Bring your completed Athletic Participation Form (attached) with you to Rehab United. Anyone trying out for sports in 2012-13 needs a new physical exam dated May 1, 2012 or later. It is highly recommended that you make a copy of your Athletic Participation Form for your files. |

Athletic Participation Forms for 2012-13
Athletic Participation Forms Friday, July 20th is the deadline for submitting Athletic Participation Forms for fall sport tryouts. Please submit your forms as soon as possible to allow processing time before your tryout date. Double check to make sure the form is complete with all signatures and your Tdap immunization date. It is recommended that you make a copy of your Athletic Participation Form for your records. Coach Engle is the contact person for Athletic Participation Forms and athletic eligibility. 2012 EARLY ASSESSMENT PROGRAM (EAP) FOR GRADE 11
In late April and early May 2012, during the California Standardized Testing and Reporting (STAR) testing in English and Algebra II or Summative High School Math, there will be a brief set of optional multiple-choice questions that are part of the Early Assessment Program (EAP). |

The SDUSD complies with state law requiring use of effective and least toxic pest management practices, and provides annual notification to all staff and parents/guardians of students enrolled at any district school with a listing of all pesticides that may be applied at any school site. |
